"Snap It & Send It" is unique nationwide call to action to bring attention to thousands of forms that fail to recognize the diversity of LGBTQ families.

You can help us fix the antiquated forms that you come across every day - at school, soccer clubs, girl scouts, day care providers, and doctors' offices. Using this app and your mobile device, simply snap a photo of the form and hit submit - that's all you need to do!

The app also contains useful information, such as sample letters to send to doctors, asking them to update their forms.
