BIDMC Health Care Proxy

Easily complete, e-sign and share a health care proxy

Health Care Proxy is a mobile application that allows patients to complete their health care proxy form and sign it electronically. It can be saved to a mobile phone or shared with others. This is meant to send to BIDMC in advance of upcoming appointments and admissions.

-This application allows the key information to be entered, which will then populate a standard Health Care Proxy form with a BIDMC logo

-The mobile app will allow the patient, proxy and witnesses to electronically sign on the same mobile device

-The health care proxy can be shared with more than one person if desired
