1. This is not a standalone app, you need to download latest amazing GO SMS Pro in order to apply this theme.

2. To apply the theme within Go SMS: menu-> themes-> local -> Ghost.
3. About permissions: the theme does not contain malicious activity, viruses, or spyware. Permission to access the Internet only need to display a small advertising banner, which perhaps you will never see, or see only once when you click "open".
