Voluntary Reporting System (Hazards / Incidents)

International Civil Aviation Organization (ICAO) Annex – 19 & DOC 9859 requires from all contracting states to establish a mechanism of Collection & Analysis of Safety Data in relation to Aircrafts and Airport Operations.

In order to comply state obligations as per aforementioned ICAO Docs

Safety Data Collection and Processing Systems (SDCPS) software has been developed at National level to capture, store, aggregate safety data / information and enable the analysis to support their safety performance management activities in the interest of flight safety.

Below mentioned two core requirements have been catered in a single module of this SDCPS Software:

- Voluntary Incident / Accidents (Occurrence) Reporting

- Voluntary Hazard Reporting

Expedient reporting will also helps PCAA to process the safety related issues in an efficient and timely manner before it leads to any incident /accident.

Hazard : A condition or an object with the potential to cause or contribute to an aircraft incident or accident

Occurrence or Incident / Accident : This is an outcome of any Hazard

Hazard Identification / Reporting Sources

Hazard identification / Reporting includes :

- Mandatory Occurrence Reporting

- Voluntary Occurrence Reporting

- Safety Occurrence Investigation

- Safety occurrence trend analysis e.g. flight data analysis (FDA)

- operational oversight (Safety Surveys & Safety Audits)

- State investigation results of accidents and serious incidents

- Information-exchange practices between Airline Operators / Aviation Service Providers & General Public.
